    Important Note - Be sure to talk to every NPC when you see their Icon in the teleportation menu, this means they have new dialogue, and this will advance their storylines!

    After unlocking the Gold Coin Fruit Tree, you will see that Polendina (Antonia's Puppet Butler) wants to talk to you regarding Antonia's condition. At the end of chapter 7, after defeating "Champion Victor", return to Hotel Krat and you'll see that Giangio wants to talk to you. He will be able to create a cure for Antonia for 1 Gold Coin Fruit. After getting the cure, talk to Polendina, and choose the option to administer the cure, then talk to Antonia.

    In chapter 11, after defeating "Laxasia the Complete", head over to Hotel Krat. Talk to Polendina, and go upstairs to Geppetto's office where Polendina was. You'll find that she has passed, and has left behind some items on her wheelchair. That is the end of her story and the achievement will unlock.

    Spicy Eggroll1@Z1PPYZ3NO - Sorry to hear about that, did you talk to them whenever they had new dialogue, when their icon was on the teleport menu? I’ve been told people have been locked out of the other NPC quest lines as well by missing these interactions.

    But for this particular one, it may be mandatory to talk to Polendina first as I mentioned, as he tells you that she has passed. It’s kind of finicky of having to talk to a certain NPC first and then the other.

    Z1PPYZ3NO@Spicy Eggroll1 Yeah i made sure to do everything. I'll maybe just try to do a NG after i'm done with all the other achievements if i still haven't gotten it

    ShailRaVeNActually, for me this achievement opened just after Polendina informed and finished dialogue about Antonia passing away. I didn't need to go to room and open letter, although it was there and I read it.
