Ollie is diabetic, and will need injections of glucose regularly--otherwise his health and stamina are severely impacted. Hence, unlocking this achievement comes naturally. Unfortunately, these injections must be crafted using empty needles and honey. Ollie will find an ample number of the former in his wandering, but acquiring honey is a bit of a problem, especially early in his playthrough. Ollie can raid bee hives early on, but it is a bit tricky, as he must first get the bees attention, then lead them some ways away from the hive, then run back to the hive and harvest the honey comb before the bees return. Later, Ollie will have the opportunity to craft or find a padded suit, or spiked suit, or fireman's coat, any one of which will protect him from bee stings. Ollie can also rob homes in the village, and occasionally find honey there, but he's not that kind of guy, is he? He can also knock out little old ladies, as some of them carry honey, but really, how low can he go?
