    Actions
    You see the piano at the Krat hotel early on in the game and even get to play some notes on it. But the achievement won’t pop until the very end of the game. After getting the Real Boy ending, instead of going into New Game+, head to the ground level of Hotel Krat and interact with the piano again. This time, you’ll be rewarded with The Pianist of Krat achievement. (Source: gameskinny.com)

    LordacrisThis isn't 100% correct you can play the Piano once you've completed either ending. When you get the option to start new game+ just hit cancel and go play the piano. It also remembers your progress if you do happen to move into NG+ - just use the piano once you reach the hotel again.

    ExraSpectreTechnically is missable, as if you go into NG+, you have to complete it again (at least if you are on PC, not sure about consoles.)

    Actions
    After getting any ending, cancel the option to go to New Game Plus, you will be back at the hotel where you have to interact with the piano. A cutscene will playout and you will get the achievement.
