    There are 33 villagers to give gifts to by the end of the game and I found this great graphic from AnnaLann12 on reddit:

    Some notes:
    • The guitar is actually at Cliffside Beach
    • "Cave Farm" in the graphic is the Cat Farm
    • Anything marked as the 'General Store' might actually be available in the vending machine in front if the General Store instead of inside.
    • Some of the items that can be found in the world won't show up until the villager requests it (pilot jacket, guitar, wheelbarrow, etc.) but some will always be there (tool belt).
    • You can find Ruby the witch around your village and it seems she sells different items during the morning versus the afternoon, so vary the times you check her stock.

    OtakuPaulI held onto the last item to give to someone and when i give it to them the achievement doesnt pop did anybody do the same or was everybody already at 5 hearts with everyone before the patch

    rosewoodjAfter I raised the last person to 5 hearts, it popped after ~10 minutes of doing nothing. (I was notified by my mobile device, and the TA scanners also picked it up. The Xbox only knew about it after a restart.)

    ArchSorceressI was hesitant to sleep too. Saved the last 2 gifts for Lily when I learned the achievement wasn't working. But I had an inkling it needed a game state change or something to trigger the achievement so I took the bus to The Gardens, which worked.
