In order to get this achievement you must be playing as the Dark Urge origin. You can find it in the origins in the create character menu, next to the companions.

After long resting a few times in the Shadow-Cursed Lands, you will be commanded by Fel to murder Isobel, the Selunite cleric keeping Last Light alive, I had already saved her at this point but still was able to do it. This will trigger the same fight that failing to save her does, so be prepared for a hard fight. Should you refuse to kill Isobel for long enough Fel will return and demand you kill your closest companion. You can do either of these, but you have to do one of them. Upon finishing, long rest again and Fel will reappear and the achievement will pop when you wake up from the long rest.

To my knowledge no actions you make in chapter 1 can lock you out of this but refusing to kill the targets will.

MGaskellYou can actually get this in act 3 aswell, when you duel Orin I chose “Show Orin the splendor of Bhaal’s favourite: Call upon the Slayer.” and got this.

AmenagannomI just popped this one by surprise yesterday in Act 2 in a Long Rest cutscene.

I started as the Dark Urge - Paladin in my buddy's playthrough where he wants to be as evil as possible. We've pretty much just selected every mean or attack-inspiring option throughout our game. We smashed the lute of Alfira, who came into our camp to say she was thankful for our action. I chose to kill her that night during a Long Rest. Sceleritas Fel then appears as my butler and encourages my evil path moving forward.

The event that triggered it was we were in camp during a Long Rest and Sceleritas Fel asked me to kill Lae'zel in her sleep. I agreed and when I awoke, Astarion and Karlach, who were in our camp, got mad about it and we killed them to stay par with our evil course. We then did a long rest, which prompted the cutscene where Sceleritas Fel appears again and turned me into the Slayer, which was cool as hell. Hope this helps!

EveryBaseJust want to add so you don't make my mistake. You need to be the one to kill Isobel so make sure you check for the quest update immediately after and make sure it counted. I had thought I did the final blow and I must have knocked her out or something and it did not count so I ended up wasting an hour killing the town just to have to do it all over again to spare having to kill my romance partner instead. It sounds like a no-brainer but you can get pretty caught up in what is going on and miss the notification.
